Where can forms appear?
| Location | How the form appears |
|---|---|
| Before login (gate page) | Customers who click your store's "Log in" link fill in the form before going to the login screen |
| After login | Shows the 「会員情報のご登録」 (Member information registration) block on the "Orders" page in customer accounts, and takes customers who haven't filled in the form to the member information page right after they log in. You need to place the 「FormSync 会員情報フォーム」 (FormSync member info form) block in the customer accounts editor |
| Profile | Shows the 「会員情報」 (Member information) block on the customer account profile, where customers can update saved values at any time |
| Thank you page | Collects post-purchase surveys and similar information |
| Order status page | Shows saved values and collects additional information |
You can also place the 「お知らせの受け取り」 (Receive notifications) block on the customer account profile, where customers choose whether to receive notifications by email and SMS. This block is available on all plans and doesn't count toward submissions.
On the form creation screen, each location shows where the form appears and whether a block is needed. Before login (gate page) needs the app embed in the theme editor. The other locations need a block placed in the customer accounts editor or the checkout editor.
What's the difference between before login and after login?
Shows the form on the gate page. After a successful submission, the customer continues to Shopify's login screen.
Right after logging in, the customer goes from the block in customer accounts to the member information page and fills in the form there.
When you create a form, you can choose "Before login (gate page)" directly as the location. To switch after creating the form, choose before login or after login (default) under "When to show this form" in the "Display timing" tab. You can't switch this for other locations.

How do I set up a form to appear before login?
Showing forms before login is available on Basic and higher plans.
- Check your themeCheck that your published theme supports Online Store 2.0.
- Turn on the app embedIn the left menu of your Shopify admin, click Online Store to open the Themes page. Click "Edit theme" (shown as "Customize" on some screens) on your current theme to open the theme editor. In App embeds on the left, turn on "FormSync login gate", then click "Save" in the top right. Do this in your current (published) theme. When "Send the "Log in" link to the gate page" in the "FormSync login gate" settings is on (the default), customers who aren't logged in are taken to the gate page when they click your store's "Log in" link. In stores that have used the app for a while, the value saved in the theme is used as is.
- Set display timing to "Before login"In the "Display timing" tab, choose "Before login".
- Check the gate pageCopy the gate page URL shown under "Store" in "Settings", then open it in your store to check it.
Values entered before login are saved temporarily. Once the customer finishes logging in, the app confirms the email address matches and applies the values to customer metafields, standard fields, and tags. Temporary data for customers who never log in is deleted after 7 days by default. When the gate page can't be shown, customers go to the normal login.
What can I set for forms shown before login?
- Don't ask customers who already passed to fill it in again on return visits
- On by default. You can set how long the passed status lasts, from 5 to 10,080 minutes (60 minutes by default).
- Behavior when the limit is reached
- Choose what happens when submissions stop because the monthly submission limit is reached. "Fail closed" (default) shows a message that the form is currently unavailable and doesn't let the customer continue to login. "Pass through" skips the form and lets the customer continue to login. If you choose pass through for a form with allow list matching, a warning is shown.
- Fallback display
- Shows the same form right after login to customers who logged in without going through the gate. Available on Standard and higher plans.
What happens to customers who log in without going through the gate?
Logging in directly without going through the gate page can't be fully prevented. If you also want customers who skipped the gate to fill in the form, turn on the setting "Show the same form right after login to customers who bypassed the gate (fallback)". This setting is available on Standard and higher plans. Customers who already filled in the form at the gate won't see the same form twice.
Can I change the login screen itself?
No. The login screen for new customer accounts is provided by Shopify. FormSync can't change its content or add fields to it.
